Complete the Form of Authority and the Terms of Instruction forms and return them to us at the address below ensuring that you:
Sign and date both the Letters of Authority and Terms of Instruction forms.
- 2 Letters of Authority per account please
- We only require 1 terms of instruction per client.
For claims against more than one provider or account, please repeat step 2. Please note that Joint Accounts require both account holders signature. |
